Vital Records

To obtain a certified copy of a vital record:

  1. Click here (https://ucrod.permitium.com) and follow the instructions to submit and pay for a Vital Record request online.
  2. Apply in person at 500 North Main Street, Suite 239, Monroe NC (Union County Government Center, 2nd Floor.)  A valid State ID or Driver’s License from DMV, Military ID or Current Passport is required. 

You may pay by all major credit/debit cards online and when applying by person.

We no longer accept requests via email, fax or mail. 

Under North Carolina law (G.S. 130A-26A), it is a felony violation to fraudulently request a copy of a vital record or obtain, possess, sell, furnish, use or attempt to use for any purpose of deception a North Carolina vital record.

North Carolina General Statute130A-92 specifies who is entitled to obtain a certified copy as follows:

  • Yourself
  • Spouse
  • Brother/Sister
  • Child/Step-child
  • Parent/Step-parent
  • Grandparent/grandchild
  • Authorized agent, attorney or legal representative (proof required)
